Physical Therapy at Home in Sussex County
Not everyone can get to a clinic when they need PT most. After a joint replacement, a fall, or a significant surgery, traveling to an appointment is the last thing that feels manageable. That gap, between when someone needs physical therapy and when they can realistically get to a clinic, is what our home visit program is built to fill.
We offer physical therapy at home across Sussex County for patients who need care on their terms. We come to your home, perform a full evaluation in the environment where you live, and start treatment without you having to go anywhere. From there, as you improve and mobility increases, care transitions to our Franklin clinic for ongoing treatment.
Home visits are an additional service that complements the clinic. They’re a way to start recovery on time, then move into the clinic where a wider range of treatment options is available.
Home visits are currently available to patients with Medicare and Medicare Advantage plans. Patients with other insurance types will begin care at our Franklin clinic.
How Home Physical Therapy Works at Franklin Physical Therapy
Home visits at Franklin Physical Therapy are not a lesser version of clinic-based care. We bring everything needed to conduct a full evaluation and provide hands-on treatment in your home. The assessment is thorough, the treatment is hands-on, and the plan of care is the same quality as what a patient receives in the clinic.
A typical home visit includes evaluation and documentation, manual therapy and hands-on treatment relevant to your condition, exercise instruction and a home program to keep things moving between visits, and planning for the transition to the clinic when you’re ready.
Most home visit patients are seen 2 to 3 times per week in the home before transitioning to the clinic for the next phase of recovery.
Transitioning to the clinic isn’t a requirement. For patients whose recovery is best suited to continue at home, whether due to distance, mobility, or an ongoing medical need, we’re glad to continue care in the home for as long as it makes sense.
What to Expect from Home Physical Therapy
Your first home visit is a full evaluation. We come to your home at a scheduled time, review your history and current condition, and get a clear sense of what treatment will look like. Home visits typically run 45 to 60 minutes.
The number of home visits before transitioning to the clinic varies based on the condition and how quickly you progress. Most post-surgical patients transition to the clinic within 2 to 4 weeks. As soon as getting to the clinic is manageable, the switch typically makes sense, since the clinic offers a wider range of treatment options.
Frequently Asked Questions
Who is a good candidate for home physical therapy?
Patients who’ve recently had surgery and can’t drive or travel comfortably, patients with limited mobility due to pain or a neurological condition, older adults who struggle with transportation, and patients who want to start care before they’re ready to commute. If you’re not sure whether a home visit makes sense for your situation, call us and we’ll talk it through.
What conditions do you treat at home?
We treat the same conditions at home as we do in the clinic, including post-surgical recovery from knee, hip, and shoulder procedures, back pain, general deconditioning, fall prevention, and more. Some specialized treatments and equipment are more practical in the clinic, which is one reason we transition care there as you progress.
How does the transition from home visits to the clinic work?
It’s a natural handoff that happens when you’re ready. We monitor your progress at each home visit and when getting to the clinic is realistic, we’ll let you know. Since you’ve been working with us the whole time, there’s no need to re-explain your history or restart care. You pick up at the clinic right where the home visits left off.
